Home Loan Evaluations

Environmental Health Specialists inspect individual water supply systems and on-site sewage disposal systems to evaluate the systems for compliance with applicable laws and sanitary standards. The seller, the buyer, or the lender may make requests for evaluation.

To request a home loan evaluation, complete the home loan evaluation request form and submit it to the Putnam County Health Department by mail or fax to (304) 757-7287. An Environmental Health Specialist will contact the requester to schedule the inspection.
