Hometown Senior Center COVID-19/Flu Shot Clinic

PCHD will be at Hometown Senior Center on Tuesday, Nov. 14, providing 2023-2024 seasonal COVID-19 shots and high-dose flu vaccinations. Senior community members are encouraged to stop by between 11am and noon to get vaccinated.

Please bring your insurance/Medicare information with you. To complete the flu vaccine registration form in advance, click here. Copies of the form will also be available upon arrival at the Center that day.
